■自宅近くに食料品店がないと認知症リスク高まる 車を利用しない高齢者２万人を調査

　日本老年学的評価研究機構は18日、自宅近くに食料品店が「全くない」と答えた高齢者は、「たくさんある」と答えた高齢者より認知症リスクが１・７倍高いとする研究成果を発表しました。店を選んだり献立を考えたりするなどの刺激が、認知機能によい影響をもたらしていると推測されるといいます。論文は国際学術誌に掲載されました。　
　東京医科歯科大学の谷友香子助教（公衆衛生学）らの研究チームが2010年から３年間、愛知県や北海道など全国の15市町に住む要介護認定を受けていない65歳以上の高齢者約５万人を追跡調査しました。
　外出時に「自分で車を運転する」もしくは「家族の車に同乗したりする」という場合は「車の利用あり」（約３万人）、どちらも該当しない場合は「車の利用なし」（約２万人）に分類し、その上で、外出時に車の利用がない高齢者に「家から１キロ以内に新鮮な野菜や果物が手に入る商店・施設はどれくらいあるか」と質問し、「たくさんある」「ある程度ある」「あまりない」「全くない」の４つの選択肢を用意しました。
　３年間で認知症になったのは、「たくさんある」と答えた高齢者が４・８％だった一方、「全くない」と答えた高齢者は９・９％でした。
　年齢や性別などの影響を調整した後の認知症リスクは、「たくさんある」に比べ、「ある程度ある」が１・２倍、「あまりない」が１・４倍、「全くない」は１・７倍となりました。一方、飲食店やコンビニ、公民館についても類似の調査をしましたが、大きなリスク差はみられなかったといいます。
　谷助教は、「買い物にゆく機会を持つことが認知症予防に重要。近くに食料品店が多くあることで外出頻度や歩行時間も増え、運動不足や社会的孤立などのリスク要因が間接的に低下しているのだろう」と推測しています。
