　厚労省によると、ロット番号は「3004734」で、モデルナが委託するスペインの製薬会社ラボラトリオス・ファルマセウティコス・ロビの工場で製造されました。製造機器から生じたステンレススチールの破片が混入した「3004667」と同時期に同じ工程で製造されたため、国内流通を担う武田薬品工業が自主回収しています。
　死亡したのは49歳の男性で、８月11日の２回目接種で使用。翌日朝に死亡が確認されました。コンピューター断層撮影（ＣＴ）による画像診断では脳出血や心臓などの異常所見はなく、近く解剖して死因を調べます。基礎疾患はなく、そばアレルギーがあったといいます。このロット番号のワクチン接種後には、38歳と30歳の男性も亡くなっています。
　武田薬品工業とアメリカのモデルナは７日、異物混入の疑いがあるモデルナ製の新型コロナウイルスワクチンで接種後の３例目の死亡事例が確認された問題について、「厚生労働省と協力し、緊急性および透明性をもって誠実に調査する」との姿勢を示し、「現時点で、健康被害や重大な安全上のリスクをもたらす根拠は認められていない」としています。
