■新型出生前検査、小規模診療所などでも実施可能に 学会の指針改定 [健康ダイジェスト]

 日本産科婦人科学会(日産婦)は20日、妊婦の血液からおなかの赤ちゃんのダウン症など3つの染色体異常を調べる新型出生前診断(NIPT)について、診療所など小規模な医療機関でも受けられるように指針を改定したと発表しました。
 妊婦の支援体制を整えることなどが条件で、これにより全国で70カ所ほど認可施設が増える可能性があるといいます。厚生労働省に報告した上で、正式に決めます。
 新たな条件としては、日本産科婦人科遺伝診療学会が主導する認定制度に合格した産科医がいる、日産婦など4学会でつくった説明文書を使う、日本小児科学会が認めた小児科医と連携し相談に応じられる、検査の実施前後に自由に小児科医に相談できる窓口がある、結果が陽性だった場合に遺伝の専門医が出張するなどしカウンセリングする仕組みがある、などを満たすこととしました。
 記者会見で三上幹男・日産婦倫理委員会委員長は、「昔の指針では基幹施設だけだったが、それだけでは困った妊婦が大勢いて、きちんとしたものを提供できなかった。基幹施設にプラス連携施設をつくるようなもの」と話しました。
 新型出生前診断は国内では2013年に始まりましたが、人工中絶などの重い選択につながるため、日産婦などが定めた条件を満たした全国109カ所の大学病院などでのみ認められてきました。
 ただ、無認可で独自に検査をする施設が増え、陽性の判定結果だけを伝え、その前後の相談には応じなかったり、この検査だけで診断が確定するわけではないことを伝えなかったり、トラブルも報告され始めた。
 日産婦は、こうした施設に妊婦が流れないよう認可施設を増やす方針案を2019年3月に公表。これに対し、日本小児科学会や日本人類遺伝学会が「不十分な体制の下に安易に行われるべきではない」として、慎重な対応を求めていました。
 新型出生前診断は、原則35歳以上の妊婦が対象。認可施設でつくる「NIPTコンソーシアム」の集計では、2019年3月までに約7万2500人が検査。うち胎児に染色体異常がわかった妊婦約1150人の約8割が、中絶しました。
 NIPTコンソーシアムの調査では、ウェブ上で確認できる認定外施設は2019年10月時点で55カ所ですが、水面下で実施している施設もあり、もっと多いとみられます。晩婚化や不妊治療の発達で高齢妊娠が増え、検査のニーズも高まってきた2016年ごろから増え始めました。
 美容外科などがインターネット広告を出し、検査結果を郵送するなどの手軽さを売りに、妊婦を呼び込み始めました。NIPTコンソーシアムが電話やホームページで調べた推計の診断数は3カ月で3500件程度。日産婦は、認可施設より倍以上も診断数が多いとみています。
 厚労省は昨年10月に作業部会を立ち上げ、無認可施設も含めた国内の検査の実態の調査を開始。結果は、今夏にも公表される見通しです。

Fewer state jobs facing the axe as government hands jobs to public sector: Government

THE STIR of a new government will see almost 80,000 fewer state and local government jobs at a time when job security has been slashed by 2 million over the past 10 years.

New Treasury figures indicate the public sector is losing jobs by nearly 6,000 on average a year, with the government blaming cuts in education, hospital and housing.

The report by the Department for Business, Energy and Industrial Strategy comes as the economy struggles to catch up with the costs of rising energy bills, consumer debt and the housing bubble.

The public sector is now "worse off than it has ever been", says the authors of the report.

"Unemployment has been rising since 2008 while the average household debt is increasing by almost 120 per cent of its pre-recession peak. This in the face of relatively stable public sector income and growth," it states.

"The sector's share of national disposable income – which measures how much income households receive from earnings of goods and services – has grown in a negative way from 14 per cent in 1993 to 22 per cent in 2013.

"In contrast, total debt – an estimate of how much a household owes on its household property, savings, business, credit cards and other debts – has fallen in a positive way from its peak in 1987."

The Department for Business said the figures suggested that the public sector was "worse off than it has ever been in the past 15 years, with high levels of state employees having less job security and less opportunities for promotion and promotion opportunities".

"There has been an increase in the number of state-run enterprises, particularly for public housing projects, while the number of local government employees is falling," it added.

Labour will today launch a new political action plan called "We need a strong public sector - to protect jobs".

Shadow Business Secretary Chuka Umunna, who is shadow business secretary, said: "The government has failed to make clear how they intend to ensure public-sector jobs are protected.

"It could put public sector staff and the public at greater risk because it will leave some struggling employees, or they may suffer a pay cut which is beyond their means."

'Tight end'

Former Tory education secretary Owen Paterson, who left Downing Street two months ago, said he was "not sure how long these predictions will last", adding that public sector employment was on "tighter" ground.

Parkside's Nanny pilot program cost taxpayers $14,000 but failed to help one mother despite her children being severely mentally ill

Queensland parents who paid $14,000 to take their two young children on an "adopt-a-neighbour" program can no longer rely on the service, a royal commission has heard.

It found that a cost saving measure meant Nanny Pilot was expensive failure and that the program's main purpose was to help vulnerable children.

One mother who was placed on the program and managed by Royal Children's Hospital, was unable to keep the youngest child after a serious injury caused her to develop post-natal depression.

The girl's mother, whose child was the same age as the girl and had suffered severe injuries, now suffers depression and suicidal thoughts.

Her husband, who was the second-in-command of the program and was at the hospital when she took her children, now suffers from an illness that limits him to bed rest in rehabilitation beds.

'The program was good'

The Nanny Pilot parents took their kids to an Adelaide hospital after a fall and when they returned to Port Chalmers, their son fell ill.

A nurse took them to hospital. After being brought to Port Chalmers, they drove their children across the state to the Victorian city of Manly, where the girl fell pregnant.

Royal Children's Hospital staff agreed a trip to Port Chalmers provided good insurance and was in line with regulations.

The study found Nanny Pilot and Royal Children's Hospital had no control over parents' ability to travel to and from the region and the parents had to travel to Port Chalmers, Melbourne, Port Hedland, Port Arthur and Port Elizabeth.

'They were completely unprepared'

One nurse was paid more than $1000.

She said she did not have any idea what to expect when her duties began, and she was expected to be a reliable pilot to the children.

The survey revealed nurses often took out loans that could not be repaid, while some nurses had trouble with payments due to insurance problems.

It also uncovered some children's nurses were involved in the program and one had been involved for years before becoming a nurse.

A nurse was "frequently" in contact with the parents but she was rarely contacted by Royal Children's Hospital.

This lack of communication and the inability to contact nurses after the parents had arrived in the regional airport resulted in the children falling ill in Australia.
